About the Role

We are seeking a passionate and dedicated individual to join our team as a Housing Stability Case Manager. As a key member of our team, you will play a vital role in supporting families in gaining and maintaining housing stability.

Key Responsibilities
  • Develop and implement case management plans that incorporate goals for both adults and children related to core areas: finding stable housing, increasing income, and family stabilization and support.
  • Meet face-to-face with families on caseload to pursue goal-related tasks, review progress, and address barriers to accessing services.
  • Collaborate with families and Housing Specialists to assist with housing search and placement process, including accompanying families to view units and advocating for families with landlords.
  • Network with community resources for housing, employment, emotional wellness services, training, etc., and connect families as appropriate.
  • Maintain regular communication with all families on caseload and document progressive engagement efforts with difficult-to-engage families.
Requirements
  • Associate's degree or two years of college.
  • In lieu of education requirement, a minimum two years of experience with homeless services programs or provision of social services is required.
  • Bachelor's degree preferred.
  • BSW and MSW degree holders must have a LSWA/LGSW or appropriate social work license prior to starting.
  • Minimum of two years of experience working in housing/homelessness or social services field.
  • Strong knowledge of community resources or ability to become knowledgeable required.
  • Strong interpersonal and organization skills required.
  • Strong communication skills including verbal and writing.
  • Proven ability to submit timely, detailed, and accurate case notes.
