Job Summary:

The Service Coordinator assumes responsibility for coordinating programs and services to help residents maintain a good quality of life and age in place. This role involves assessing and identifying resident and family needs and outside resources to meet those needs.

Key Responsibilities:
  • Network with community organizations to identify affordable and new services to offer residents.
  • Conduct psychosocial assessments to evaluate current condition and identify potential adjustments.
  • Provide general case management and referral services, maintaining complete and updated resident files.
  • Coordinate services for optimal living, including aging in place and wellness philosophies.
  • Develop and implement Care Plans and Quarterly Monitoring with residents needing referrals.
  • Submit Semi-Annual Performance Reports to maintain funding for the Service Coordination program.
  • Engage residents to identify areas of need and make necessary referrals, promoting resident involvement in on and off-site programs.
  • Assist in coordinating home care services to enable independent living and aging in place.
  • Coordinate counseling, health screenings, and legal advocacy services.
Requirements:
  • Minimum degree in social work, psychology, or gerontology.
  • Two or more years of experience in social service delivery with elderly and/or family populations.
  • Demonstrated working knowledge of supportive services and resources in the area.
  • Excellent communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and resident software data systems.
