Position Summary
The Senior Services Coordinator plays a vital role in managing and facilitating service coordination and case management for residents in a senior affordable housing community. This position aims to empower residents to maintain their independence while fostering connections within the community. The Coordinator will work closely with residents, community councils, and service providers to assess needs, connect individuals to resources, and promote health and well-being through educational initiatives and community engagement.
Key Responsibilities:
- Collaborate effectively with residents, community councils, and committees.
•Conduct comprehensive psychosocial assessments for residents who may be vulnerable or in need of support.
•Assist prospective residents in understanding their current and future needs.
•Provide case management and referral services as necessary.
•Maintain accurate and updated records of residents and community programs through a digital case management system.
•Participate in regular team meetings and provide updates on resident status and program effectiveness.
•Build and maintain relationships with local service providers to ensure quality support for residents.
•Ensure all service providers meet established criteria before referrals are made.
•Support residents in developing informal support networks with peers, family, and friends.
•Engage in ongoing training and evaluation to enhance service quality for residents and the community.
•Act as a liaison between community organizations, service providers, and residents.
•Design and implement engaging educational programs and group activities.
- Maintain a comprehensive directory of community resources for residents and their families.
Qualifications:
Education and Experience:
- Associate degree or higher in social work, psychology, gerontology, counseling, or a related field, with at least 1 year of experience working with older adults in a community setting; or 3+ years of experience in case management or service coordination with older adults.
•Familiarity with senior community resources and services is essential.
•Proven ability to work independently and achieve positive outcomes for older adults.
•Strong oral and written communication skills are required, along with proficiency in Microsoft Office applications.
