Client Support Case Coordinator

Position Overview:

The Client Support Case Coordinator plays a crucial role in delivering enhanced primary care and case management services to designated clients.

Core Responsibilities:

  • Gather, assess, and organize essential reports by established deadlines.
  • Oversee and maintain comprehensive records for clients.
  • Acquire medical documentation for all clients and aid in medication oversight.
  • Track clients' health conditions and overall well-being, ensuring prompt resolution of medical and social concerns.
  • Assist in arranging medical appointments and adhering to healthcare providers' recommendations.
  • Engage with healthcare professionals for holistic case management.
  • Document client data using Therap by the conclusion of the service month.
  • Facilitate and coordinate related services such as transportation, appointment scheduling, counseling, and additional support.
  • Perform other assigned tasks as necessary.

Qualifications:

Required: A Bachelor's degree in a relevant discipline or equivalent experience. Previous experience working with individuals with disabilities is preferred.
