Schedule: Monday - Friday 8:00am-4:30pm

Job Summary:

The Community Care Coordinator plays a vital role in our expanding organization, providing ongoing case management services. Our mission is to foster independence and improve the quality of life for individuals through advocacy, education, and client-centered services. We are actively engaged in community initiatives, including various events and programs. The Community Care Coordinator will manage all aspects of ongoing case management for both waiver and non-waiver programs, ensuring access to high-quality home and community-based services in a timely and effective manner.

Key Responsibilities:

  • Build and maintain strong interpersonal relationships.
  • Demonstrate empathy and understanding of individual needs.
  • Assist clients with the Medicaid application process.
  • Adhere to relevant manuals and comply with all applicable laws.
  • Support long-term care planning and goal setting.
  • Assess program eligibility using established screening tools.
  • Maintain knowledge of State & Federal regulations.
  • Complete additional tasks as assigned.

Qualifications:

  • Continuous employment as a Care Manager with an Area Agency on Aging since June 30, 2018; or
  • A registered nurse, licensed practical nurse, or an associate degree in nursing with at least one year of relevant experience; or
  • A Bachelor's Degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services; or
  • A Bachelor's Degree in any field with a minimum of two years of direct service experience with older adults or individuals with disabilities; or
  • A Master's Degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services; or
  • An associate degree in any field with at least four years of direct service experience with older adults or individuals with disabilities.
  • Ability to lift up to 25 lbs. occasionally.
