As the leading provider of services for victims of domestic violence in Albany County, our Domestic Violence Programs ensure safety for all victims, including the elderly, while offering the necessary support, resources, and tools to help them escape abusive situations and regain control of their lives. Our services are inclusive, available to all victims and their dependent children, irrespective of gender, age, ethnicity, or sexual orientation.
Position Overview: The Case Manager is responsible for delivering tailored case management services to clients enrolled in the Project Break Free rapid re-housing initiative. This role involves assisting clients in securing safe and affordable permanent housing, connecting them to essential services, providing accompaniment to various agencies, and offering comprehensive support, education, and guidance.
Key Responsibilities:
- Evaluate client needs and capabilities to determine the appropriate level of support required.
- Accompany clients to various agencies, including DSS and law enforcement, advocating on their behalf through both in-person and written communication.
- Maintain precise case documentation and fulfill all necessary paperwork and statistical requirements for the organization and funding sources.
- Collaborate with clients to develop personalized safety plans.
- Assist clients in locating safe and affordable housing options and facilitate their transition into these residences.
- Establish and nurture positive relationships with community services and landlords.
- Engage in staff meetings and individual supervision sessions.
- Report any suspected cases of child abuse or maltreatment as mandated.
- Uphold the mission of Equinox and the Domestic Violence Services Program in all actions.
- Demonstrate crisis intervention skills effectively.
- Provide objective and supportive listening and guidance to clients.
- Communicate clearly and concisely in both written and verbal forms.
- Work independently while also being an effective team member.
- Exhibit calm and cooperative behavior in all interactions.
- Prioritize and manage multiple tasks in an organized manner.
- Ensure strict confidentiality of client information while engaging with a diverse clientele.
- Perform additional duties as required to enhance and sustain programming.
Physical Requirements:
- Ability to navigate stairs.
- Capability to lift and carry a minimum of 15 lbs.
- Ability to respond to basic facility emergencies.
- Must possess a valid NYS Driver License to transport clients using agency vehicles.
Minimum Qualifications:
- Bachelor's Degree in a human services field (5 to 7 years of relevant experience may substitute for an advanced degree).
- A minimum of two years of experience in human services.
- Possession of a clean, valid NYS Driver License.
Preferred Qualifications:
- At least one year of experience in case management.
- Fluency in Spanish or another language is advantageous.
- Prior experience working with populations affected by domestic violence, substance abuse, or mental health issues.
Work Schedule: Monday through Friday
