As the leading provider of services for victims of domestic violence in Albany County, our Domestic Violence Programs prioritize the safety of victims, including the elderly, and equip them with the necessary support, resources, and tools to escape abusive situations and regain control over their lives. Our services are inclusive, available to all victims and their dependent children, irrespective of gender, age, ethnicity, or sexual orientation.
Position Overview: The Emergency Services Case Manager is responsible for delivering crisis intervention, short-term case management, and follow-up support to victims who reach out via the 24-hour hotline, utilize the web-based chat, walk in for assistance, or are referred by an Equinox Domestic Violence team member for urgent help. The role encompasses a range of services, including supportive listening, initial eligibility assessments for Domestic Violence Residential and Outreach Services, safety planning, information and referral, emergency shelter placement, advocacy, case management, and ongoing support. The Emergency Services Case Manager collaborates with fellow emergency case managers and coordinates with all domestic violence staff to ensure clients receive comprehensive and cohesive services.
Key Responsibilities:
- Responds to the 24-hour hotline and crisis chat line, providing crisis intervention, supportive listening, and information referral; develops safety plans with clients and conducts eligibility assessments for all Domestic Violence services.
- Evaluates eligibility and options for victims seeking domestic violence shelter; conducts shelter intake when appropriate.
- Assists victims in securing alternative emergency housing options (such as out-of-county DV shelters, temporary motel lodging, or staying with family and friends), including transportation and food assistance when Equinox shelter is unavailable. Follows up on client needs.
- Provides short-term case management and follow-up support to victims who have been alternately placed, have exited shelter, or have been referred by an Equinox DV team member for emergency assistance.
- Engages in community education and outreach regarding domestic and family violence, elder abuse, and human trafficking.
- Accompanies clients to various agencies, including DSS and law enforcement, advocating for clients both in person and through phone and written communication.
- Participates in coordination and outreach efforts with other community services, including providing services at co-location sites.
- Establishes and maintains positive working relationships with other community services, including regional domestic violence programs.
- Maintains client records in accordance with program and regulatory requirements, ensuring compliance with statistical reporting for the agency and funding sources.
- Acts in alignment with the mission of Equinox, upholding the highest standards of professional ethics and ensuring confidentiality when interacting with clients, employees, and the public.
- May represent the agency in formal and informal groups related to domestic violence services.
- Adheres to all documentation requirements.
- Participates in client services coordination team meetings, staff meetings, and individual supervision sessions.
- Reports suspected cases of child abuse/maltreatment as a mandated reporter.
- Performs additional duties as assigned to enhance and sustain programming.
Physical Requirements:
- Ability to navigate stairs.
- Ability to lift and carry a minimum of 15 lbs.
- Ability to manage basic facility emergencies.
- Ability to drive and transport clients using agency vehicles as needed.
Minimum Qualifications:
- Bachelor's Degree in human services or a relevant field (2 years of relevant experience may substitute for an advanced degree).
- At least one year of experience in human services.
- Possession of a clean, valid NYS Driver License and reliable transportation.
Preferred Qualifications:
- Fluency in Spanish or another language.
- Prior experience working with populations affected by domestic violence, sexual assault, elder abuse, human trafficking, mental health issues, or substance abuse.
- Experience in case management or crisis intervention services is preferred.
