Client Support Specialist

Position Overview:
The West Side Federation for Senior and Supportive Housing is seeking a dedicated Client Support Specialist. This role involves delivering a wide array of client support services, including both individual and group interventions.
The primary objective of these services is to guarantee that each client has access to the necessary resources and support that enhance their ability to thrive within the community, fostering a sense of well-being, health, and strong community connections.
The Client Support Specialist will provide direct assistance while also guiding clients in accessing additional supportive services as required. Furthermore, this role includes the responsibility of entering pertinent information into the data systems mandated by relevant authorities.
Direct Client Services:
- Cultivate a personalized relationship with clients to evaluate their interests and requirements, facilitating the delivery of support services.
- Conduct individual meetings with clients to assess their needs and collaboratively determine a suitable course of action.
- Aid clients in securing and maintaining appropriate medical and mental health care.
- Assist clients in obtaining and sustaining entitlements.
- Provide guidance, information, and advocacy concerning housing needs and challenges.
- Support clients in establishing and/or preserving family and social connections.
- Encourage participation in organizational activities to prevent social isolation and enhance quality of life.
- Offer individual counseling as necessary.
- Facilitate group work services.
- Accompany clients to appointments when required.
- Organize and lead meetings and workshops on topics beneficial to clients.
- Provide crisis intervention services as needed.
- Create and maintain case records in accordance with regulatory requirements. All documentation must be completed within the specified time frames.
- Ensure compliance with all relevant regulations.
- Contribute to the management of client data, entering information into required systems as necessary.
Additional Responsibilities:
- Uphold a high level of respect and care for clients.
- Maintain confidentiality as required by organizational policies.
- Collaborate effectively with other staff members.
- Participate in training sessions and conferences to enhance job skills.
- Attend supervision sessions regularly.
- Engage in community meetings and collaborate with other community organizations as directed.
- Attend staff meetings as required.
- Perform other duties as assigned.
Qualifications:
- A Bachelor’s degree in Social Work is preferred; a Master’s degree in Social Work is highly desirable.
- Experience working with elderly populations and/or individuals with special needs is advantageous.
- Background in group work is preferred.
