Client Support Specialist

Overview: In the role of Case Manager at Samaritan Daytop Village, you will be instrumental in supporting individuals who need assistance with social services. Your duties will encompass assessing clients, developing Independent Living Plans, identifying suitable community resources, and facilitating clients' transition to stable housing.
Key Responsibilities:
- Build and maintain professional relationships with clients
- Conduct comprehensive intake and psycho-social evaluations
- Create independent living plans in collaboration with clients
- Guide clients in accessing permanent housing and community services
- Track and document client progress effectively
- Advocate for the needs of clients
- Provide necessary information for reporting purposes
- Act as a representative for clients with external organizations
- Coordinate and accompany clients to various appointments
- Assist clients in completing benefit applications
- Stay informed about changes in the field
- Perform additional duties as requested
- High School Diploma with over 5 years of case management experience, or an Associate's Degree with 3+ years, or a Bachelor's Degree with 2+ years
- Familiarity with community resources in New York City
- Ability to uphold confidentiality and show sensitivity to client needs
- Experience working with diverse populations
- Proficiency in computer applications, including Microsoft Office Suite and EHR systems
- Associate's or Bachelor's degree in Human Services or a related discipline
- Experience with homeless populations
- Knowledge of DHS CARES Software
- Understanding of the challenges faced by the homeless
The work setting at Samaritan Daytop Village is transitional, involving a mix of activities such as sitting, standing, lifting, and walking.
