- Guide clients in accessing permanent housing and community services
- Track and document clients' progress effectively
- Advocate for the needs of clients
- Provide necessary information for reporting purposes
- Serve as a client representative with external organizations
- Organize and accompany clients to appointments
- Assist clients in completing benefit applications
- Stay informed about changes in the field
- Perform additional duties as requested
- High School Diploma with 5+ years of case management experience, or an Associate's Degree with 3+ years, or a Bachelor's Degree with 2+ years
- Familiarity with community resources in NYC
- Ability to uphold confidentiality and show sensitivity to clients' needs
- Experience working with diverse populations
- Proficient in computer applications, including Microsoft Office Suite and EHR
Preferred Qualifications:
- Associate's or Bachelor's degree in Human Services or a related field
- Experience with homeless populations
- Knowledge of DHS CARES Software
- Understanding of the challenges faced by the homeless
