Overview: In the role of Case Manager at Samaritan Daytop Village, you will be instrumental in supporting individuals in need of social services. Key Responsibilities:
  • Build and maintain professional relationships with clients
  • Conduct comprehensive intake and psycho-social evaluations
  • Create independent living plans with input from clients
  • Guide clients in accessing permanent housing and community services
  • Track and document clients' progress effectively
  • Advocate for the needs of clients
  • Provide necessary information for reporting purposes
  • Serve as a client representative with external organizations
  • Organize and accompany clients to appointments
  • Assist clients in completing benefit applications
  • Stay informed about changes in the field
  • Perform additional duties as requested
Qualifications:
  • High School Diploma with 5+ years of case management experience, or an Associate's Degree with 3+ years, or a Bachelor's Degree with 2+ years
  • Familiarity with community resources in NYC
  • Ability to uphold confidentiality and show sensitivity to clients' needs
  • Experience working with diverse populations
  • Proficient in computer applications, including Microsoft Office Suite and EHR
Licenses/Certifications: None required
Preferred Qualifications:
  • Associate's or Bachelor's degree in Human Services or a related field
  • Experience with homeless populations
  • Knowledge of DHS CARES Software
  • Understanding of the challenges faced by the homeless
