Job Overview:

We are seeking an experienced Patient Care Coordinator to join our team at CARTI. In this role, you will work closely with patients, families, and healthcare professionals to provide exceptional care and support throughout the breast cancer journey.

About the Job:

The ideal candidate will have a strong background in patient care coordination, excellent communication skills, and a passion for delivering high-quality patient-centered care. You will be responsible for navigating patients through the complex healthcare system, addressing their needs and concerns, and connecting them with available resources and support systems.

Responsibilities:
  1. Develop and implement individualized care plans that address the unique needs and goals of each patient.
  2. Collaborate with healthcare providers to ensure seamless transitions between different stages of care.
  3. Provide education and support to patients and families, addressing their concerns and questions throughout the breast cancer journey.
  4. Identify and address potential barriers to care, developing strategies to overcome them.
Requirements:
  • High School Diploma or equivalent; higher education preferred.
  • BLS certification required.
  • Certified Navigator – Breast Advocate (CN-BA) through NCBC.
  • Breast Navigation program preferred.
  • 1 year experience in breast navigation preferred.
