">Your Role

As a Case Manager RN, you'll work closely with physicians, registered nurses, specialized departments, and community resources to develop personalized care plans that meet each patient's unique needs. You'll build trust through compassion, critical thinking, and creative problem-solving, ensuring optimal outcomes and reducing barriers to healing.

">Responsibilities
  • Conduct initial patient reviews and initiate contact to understand their clinical, spiritual, financial, and educational needs.
  • Participate in daily multidisciplinary rounds and weekly length of stay meetings to discuss and review patient care plans.
  • Develop and implement discharge plans to meet post-acute care requirements, avoid outlier days, reduce length of stay, and minimize readmission rates.
  • Collaborate with patients, families, vendors, post-acute care providers, and teammates to ensure seamless communication and care coordination.
">Requirements
  • An unencumbered license to practice nursing in the State of Arkansas.
  • Three years of acute care nursing experience.
  • A Bachelor's Degree in Nursing (BSN) or an Associate Degree with CCM or ACM Certification within two years of hire date.
  • Previous case management, utilization management, or discharge planning experience strongly preferred.
">Compensation and Benefits

We offer a competitive hourly rate of $32.75-$43.24, plus additional pay for participation in our clinical ladder program and referral bonuses. Enjoy excellent benefits, tuition reimbursement, relocation assistance, and a sign-on bonus up to $25,000 for eligible hires.
