We are seeking a compassionate and experienced Medical Social Worker to join our team at MSA Hospice of the Upstate. As a Care Coordinator, you will play a vital role in providing social work services to our patients and their families.

About the Role

This is a Full-Time position, requiring a Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education. You must be currently licensed to practice as a social worker in the state of SC and have a minimum of one-year Hospice experience.

Job Responsibilities

As a Medical Social Worker, your primary responsibilities will include:

  • Providing social work services in accordance with the physician's plan of treatment
  • Working closely with our interdisciplinary team to assess patient needs and provide support
  • Assessing grief and bereavement needs and providing counseling as needed
  • Serving as a liaison with community resources to ensure seamless care coordination
