Position Overview:
This role is pivotal in facilitating the transition of individuals re-entering society from correctional facilities. The position is focused on coordinating essential services before and after release, ensuring a smooth reintegration process for those involved with community corrections.
Key Responsibilities:
- Conduct monthly orientation sessions to inform participants about available resources.
- Provide ongoing peer support and counseling to individuals both during incarceration and throughout their reintegration journey.
- Document participant progress and interactions in the WCS Wiser Choice database.
- Connect participants with tailored community resources, including job training, educational opportunities, and health services.
- Offer continuous mentoring and coaching to participants, aiming to reduce the likelihood of reoffending.
- Collaborate with the Liaison to facilitate orientation sessions for eligible individuals interested in the reintegration program.
- Establish Recovery Support Teams to create a robust support network for participants.
- Engage in Recovery Support Team meetings to develop a Single Coordinated Care Plan (SCCP) that aligns with individual strengths and needs, reviewed every 30 days.
- Provide or arrange transportation for participants as necessary, demonstrating flexibility in meeting clients at various locations.
- Maintain a manageable caseload, ensuring regular face-to-face contact and participation in relevant meetings.
- Stay connected with participants across different community settings.
Additional Responsibilities:
- Educate and involve participants in their treatment and recovery processes.
- Participate in Recovery Support meetings as needed.
- Engage in training sessions to enhance facilitation and mentoring skills.
- Collaborate with various stakeholders, including probation and parole officers, social services, and legal representatives.
- Assist participants in accessing treatment and recovery services, including mental health support, housing, and employment resources.
Qualifications:
- Possess a Bachelor's Degree in Criminal Justice, Social Work, or a related field.
- Have a minimum of three years of relevant experience.
- Familiarity with the Wraparound Philosophy is advantageous.
- Hold a valid driver's license and have access to a vehicle.
- Demonstrate proficiency in computer skills, including typing at least 35 words per minute.
- Meet security clearance requirements as established by relevant authorities.
Work Environment:
This position involves both office work and field activities, with approximately half of the time spent in the community. Flexibility in scheduling is essential to accommodate the needs of participants.
Physical Requirements:
The role requires the ability to sit for extended periods, travel by vehicle, and effectively communicate with diverse populations. Reasonable accommodations may be made for individuals with disabilities to perform essential functions.
