POSITION OVERVIEW:

The Supports Coordinator utilizes their educational background, communication abilities, and observational skills to evaluate the functional and medical care requirements of participants in accordance with established program guidelines.

Employing a person-centered methodology, the Supports Coordinator formulates care plans to address these needs and executes the care strategies to facilitate long-term support, enabling participants to maintain their independence in a home setting.


KEY RESPONSIBILITIES:
Conduct thorough assessments of participants' health requirements.


Utilizing a person-centered framework, collaborate with participants, their chosen allies, and healthcare providers to create a comprehensive care plan.

Assist participants in determining the frequency and duration of services, implementing the plan as per participant approval.

Seek guidance from supervisory personnel as necessary to ensure the delivery of high-quality care plans.

Continuously monitor participants' conditions and the services provided, making adjustments based on reassessments or ongoing evaluations. Maintain open communication with participants' families and healthcare providers, acting as an advocate for the participant.

Maintain accurate participant records and relevant program documentation.


Stay informed on current best practices in geriatric and chronic care nursing to effectively address participants' functional and medical needs.


QUALIFICATIONS:
To excel in this role, candidates must demonstrate proficiency in all essential duties. The following qualifications are essential:

  • Valid Michigan driver's license.
  • Reliable transportation and proof of automobile insurance.
  • Possession of a cell phone.
  • Current Michigan Registered Nursing License.
  • CPR certification.
  • Tuberculosis testing upon hire and annual symptom assessments thereafter.
EDUCATION and/or

EXPERIENCE:

Bachelor of Science in Nursing or an Associate Degree/Diploma in Nursing with two to three years of relevant experience.

A minimum of one year of experience in home care is preferred. Candidates must hold a current State of Michigan Registered Nurse license and have experience working with elderly or disabled adults.

REASONING ABILITY:
Ability to identify problems, gather data, establish facts, and draw valid conclusions. Capable of interpreting a wide range of regulations and instructions while managing various abstract and concrete variables. Ability to assess environmental factors affecting a participant's well-being.

OTHER SKILLS &

ABILITIES:
Strong interpersonal skills and the ability to work independently as well as collaboratively. Familiarity with health service delivery systems and community resources is essential.

LANGUAGE SKILLS:
Ability to read, analyze, and interpret professional literature, program standards, and guidelines. Proficient in writing reports and correspondence.

Capable of effectively presenting information and addressing inquiries from participants, providers, educators, social service personnel, and the public.


MATHEMATICAL SKILLS:

Proficient in basic arithmetic operations including addition, subtraction, multiplication, and division using whole numbers, fractions, and decimals.


TECHNICAL SKILLS:
Basic typing skills, proficiency in Microsoft applications, and the ability to utilize the internet, printers, copiers, scanners, and password management systems.

PHYSICAL DEMANDS:

The physical requirements described are representative of those that must be met by an employee to successfully perform the essential functions of this position.
