Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Care team. As a Primary Care Nurse, you will provide high-quality patient care to individuals in their homes, working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Key Responsibilities
  • Provide primary nursing care to patients in their homes, following physician orders and adhering to our organization's policies and procedures.
  • Develop and implement individualized care plans in collaboration with our Population Health Case Managers.
  • Supervise and support Home Healthcare Aides and Licensed Practical Nurses as needed.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans to achieve top decile client outcomes and patient satisfaction.
  • Utilize evidence-based practices, educational tools, and pathways to optimize patient care and outcomes.
  • Participate in quality improvement activities to enhance agency performance and patient care.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
Requirements
  • Graduate of an approved Nursing education program, Bachelor of Science in Nursing (BSN) preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum of one (1) year professional nursing experience, preferably in Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit.
  • Strong computer and technology skills, including proficiency in electronic medical records.
  • Current Driver's license and reliable transportation.
