POSITION OVERVIEW:
The Care Coordinator utilizes their educational background, communication abilities, and observational skills to evaluate the functional and medical care requirements of participants in accordance with the MI Choice Home and Community Based Services program guidelines.
Employing a participant-centered methodology, the Care Coordinator formulates care plans tailored to these needs and executes the care strategies to provide long-term support, enabling participants to maintain their independence at home.
KEY RESPONSIBILITIES:
Conduct comprehensive assessments of participants' health requirements.
Utilizing a participant-centered approach, collaborate with the participant, their chosen support network, and their healthcare provider to develop a care plan.
Seek guidance from supervisory staff as necessary to ensure high-quality care plans.
Continuously monitor the participant's condition and the services provided, adjusting the care plan as needed based on reassessments or regular monitoring. Maintain open communication with the participant's family and healthcare provider, acting as the participant's advocate.
Maintain accurate participant records and relevant program documentation.
Stay informed on current practices in geriatric and chronic care nursing to effectively address participants' functional and medical care needs.
QUALIFICATIONS:
To excel in this role, candidates must successfully fulfill each essential duty. The qualifications listed below represent the knowledge, certification, skills, and/or abilities required. - Must hold a valid Michigan driver's license.
- Must have reliable transportation and provide proof of automobile insurance.
- Must possess a cell phone.
- Must have a current Michigan Registered Nursing License.
- Must be CPR certified.
- Must undergo tuberculosis testing upon hire and participate in annual symptom reviews thereafter.
EXPERIENCE:
A Bachelor of Science in Nursing or an Associate Degree/Diploma in Nursing with two to three years of relevant experience.
A minimum of one year of home care experience is preferred. A current State of Michigan Registered Nurse license is required. Experience working with elderly and/or disabled adults is advantageous.REASONING ABILITY:
Ability to identify problems, gather data, establish facts, and draw valid conclusions. Ability to interpret a wide range of regulations and instructions and manage various abstract and concrete variables. Ability to assess environmental factors in relation to a participant's well-being.
OTHER SKILLS &
ABILITIES:
Ability to engage effectively with individuals on a personal level. Ability to work independently as well as collaboratively within a team. Familiarity with health-related service delivery systems and community resources is essential.
LANGUAGE SKILLS:
Ability to read, analyze, and interpret professional literature, journals, and program standards. Ability to compose reports and correspondence.
Ability to effectively present information and respond to inquiries from participants, providers, educators, social service personnel, and the general public.
MATHEMATICAL SKILLS:
Ability to perform basic arithmetic operations in various units of measure, utilizing whole numbers, common fractions, and decimals.
TECHNICAL SKILLS:
Basic typing skills, proficiency in Microsoft applications, and ability to utilize the internet, printers, copiers, scanners, and password management systems.
PHYSICAL DEMANDS:
