POSITION OVERVIEW:

The Supports Coordinator utilizes their educational background, communication abilities, and observational skills to evaluate participants' functional and medical care requirements in accordance with established program guidelines.

Employing a person-centered methodology, the Supports Coordinator formulates care plans tailored to these needs and executes the care strategies to facilitate long-term support, enabling participants to maintain independence in their home settings.


KEY RESPONSIBILITIES:
Conduct comprehensive assessments of participant health requirements.


Utilize a person-centered approach to create a care plan in collaboration with the participant, their chosen allies, and their healthcare provider.

Assist participants in determining the frequency and duration of services, and implement the approved care plan.

Seek clinical guidance from supervisory personnel as necessary to ensure high-quality care plans.

Monitor the participant's health status and service delivery related to their care plan. Adjust the plan as needed based on reassessments or routine monitoring. Maintain open communication with the participant's family and healthcare provider. Act as an advocate for the participant.

Maintain accurate participant records and relevant program documentation.


Stay informed on current practices in geriatric and chronic care nursing to effectively address participants' functional and medical care needs.


QUALIFICATIONS:
To excel in this role, candidates must meet the following criteria:

  • Valid Michigan driver's license.
  • Reliable transportation and proof of automobile insurance.
  • Possession of a cell phone.
  • Current Michigan Registered Nursing License.
  • CPR certification.
  • Tuberculosis testing upon hire and annual symptom evaluations thereafter.
EDUCATION and/or

EXPERIENCE:

A Bachelor of Science in Nursing or an Associate Degree/Diploma in Nursing with two to three years of relevant experience.

A minimum of one year of home care experience is preferred. Must hold a current State of Michigan Registered Nurse license. Experience with elderly and/or disabled adults is advantageous.

REASONING ABILITY:
Ability to identify issues, gather information, establish facts, and draw valid conclusions. Capability to interpret a wide range of regulations and instructions while managing various abstract and concrete variables. Proficiency in analyzing environmental factors concerning a participant's well-being.

OTHER SKILLS &

ABILITIES:
Strong interpersonal skills and the ability to work both independently and collaboratively. Familiarity with health-related service delivery systems and community resources is essential.

LANGUAGE SKILLS:
Ability to read, analyze, and interpret professional literature, journals, and program standards. Proficient in writing reports and correspondence.

Capable of effectively presenting information and addressing inquiries from participants, providers, educators, social service personnel, and the general public.


MATHEMATICAL SKILLS:

Proficient in basic arithmetic operations, including addition, subtraction, multiplication, and division using whole numbers, fractions, and decimals.


TECHNICAL SKILLS:
Basic typing skills, proficiency in Microsoft applications, and the ability to utilize the internet, printers, copiers, scanners, and password management systems.
