ROLE OVERVIEW

Position Title:
Housing Support Specialist

Role Summary:

The Housing Support Specialist plays a crucial role in aiding individuals to secure and maintain stable housing while progressing towards self-sufficiency.

This position involves networking, collaborating, and coordinating various services including housing, behavioral health, substance use, and mental health support, as well as community and employment resources for program participants.

The Specialist will also facilitate both individual and group sessions, track participant engagement, and compile necessary reporting data.

Educational Qualifications:
A CASAC certification, or a Bachelor's or Master's degree in health and human services is required. Experience in case management and familiarity with individuals facing co-occurring disorders is preferred.

Professional Experience:

Candidates should have experience working with individuals dealing with substance use and/or mental health challenges, homelessness, and an understanding of community agencies and entitlement programs. Flexibility in scheduling during weekdays is essential.

Key Responsibilities:

  • Conduct housing intake evaluations to assess participant needs.
  • Assist individuals in finding safe and affordable housing, fostering relationships with landlords and community partners.
  • Support participants in securing safe living arrangements.
  • Perform housing inspections, establish rental agreements, and conduct visits to ensure proper maintenance of residences.
  • Build and maintain relationships with landlords.
  • Guide participants in developing personalized recovery plans.
  • Facilitate access to entitlement programs, medical care, mental health services, and substance use treatment.
  • Coordinate case management efforts with treatment programs and other community resources.
  • Support participants' progress by collaborating with employment specialists, training programs, and employers to achieve educational and vocational objectives.
  • Monitor participant progress through regular home and office visits.
  • Utilize harm reduction strategies and motivational techniques to provide client-centered services that promote housing stability.
  • Deliver direct care services including individual and group meetings, community gatherings, and assistance with entitlement programs.
  • Maintain accurate individual case records, collect data, and prepare reports as required.
  • A valid driver’s license is necessary.
  • Availability for flexible hours, including on-call duties, is required.
