Job Description:

We are seeking a skilled Clinical Pharmacist to join our team at Duke Health, working closely with the Rheumatology and Dermatology department. The successful candidate will provide clinical pharmacy services to patients, ensuring safe and effective medication use. With a focus on collaborative practice agreements, this role involves prescribing and monitoring medications, as well as coordinating patient care and laboratory monitoring.

About the Role:
  • Participate in clinic activities to provide clinical pharmacy services and oversight for safe medication use
  • Prescribe and adjust medications as permitted through collaborative practice agreements
  • Monitor patients for therapeutic and/or adverse responses to medication
  • Coordinate patient care and ensure appropriate follow-up with other outpatient or inpatient teams
Key Responsibilities:
  • Provide medication counseling and education to patients
  • Screen for drug interactions, therapeutic duplications, omissions, etc.
  • Facilitate prescription fulfillment through Duke specialty and retail pharmacies
  • Support Duke Specialty Pharmacy patients by serving as the on-call pharmacist on a rotating schedule
Requirements:
  • Doctor of Pharmacy Degree or BS Pharmacy Degree with 2 years of pharmacy practice experience
  • Licensed Pharmacist or eligible to practice pharmacy in North Carolina

The estimated salary for this position is $140,000 - $160,000 per year, depending on experience.
