About the Role

Southeastern Integrated Care LLC is seeking a highly skilled and compassionate Case Manager to join our Community Support Team. As a Case Manager, you will play a vital role in providing high-quality care and support to adults with mental health and substance abuse issues.

Key Responsibilities
  • Develop and implement comprehensive service plans to meet the unique needs of each individual
  • Provide psycho-education and support to individuals and their families
  • Assist with crisis interventions and provide ongoing support
  • Collaborate with community and natural supports to ensure seamless care coordination
  • Monitor and document progress, ensuring effective strategies and interventions
  • Coordinate service user transitions to other types of services
Requirements
  • Minimum of 2 years of experience working with adults with mental health and substance abuse issues
  • Valid NC driver's license and personal vehicle insurance coverage
  • Bachelor's degree in a human service field or related field
