About the Role

Southeastern Integrated Care LLC is seeking a highly skilled and compassionate Case Manager-Community Support Team to join our dynamic team. As a key member of our Community Support Team, you will play a vital role in providing high-quality care and support to individuals struggling with behavioral health disorders, substance use disorders, and chronic medical conditions.

Key Responsibilities
  • Assess client needs and develop comprehensive service plans to promote recovery and symptom reduction.
  • Provide psycho-education and support to individuals and their families, as well as assistance with crisis interventions.
  • Collaborate with the Community Support Team Leader, paraprofessional and associate professional staff to ensure seamless service delivery.
  • Monitor and document client progress, and make adjustments to service plans as needed.
  • Participate in a first responder on-call system to provide 24/7 support to consumers and their natural support networks.
Requirements
  • Master's degree in a human service field or a related field, or a Bachelor's degree in a human service field with two years of experience in adult behavioral health.
  • Valid NC driver's license and personal vehicle insurance coverage.
