About the Role

Southeastern Integrated Care LLC is seeking a highly skilled and compassionate Case Manager-Community Support Team to join our dynamic team. As a key member of our Community Support Team, you will play a vital role in providing high-quality care and support to individuals struggling with behavioral health disorders, substance use disorders, and chronic medical conditions.

Job Summary

The primary responsibility of this position is to provide Community Support Team services, including direct and indirect interventions, to individuals with complex and extensive treatment needs. You will work collaboratively with a team of professionals to develop and implement comprehensive service plans, provide psycho-education, and assist with crisis interventions.

Key Responsibilities
  • Develop and implement Person-Centered Plans (PCPs) in collaboration with the Community Support Team Leader and other team members.
  • Provide psycho-education and support to individuals and their families, as indicated in the PCP.
  • Assist with crisis interventions and provide support to individuals in crisis situations.
  • Coordinate services and linkages to other community resources, as needed.
  • Monitor and document the status of individuals' progress and the effectiveness of strategies and interventions outlined in the PCP.
  • Participate in a first-responder on-call system and coordinate resources as needed.
Requirements

To be successful in this role, you must have a minimum of 2 years of experience working with adults with behavioral health and substance abuse issues. You must also possess a Bachelor's degree in a human service field or a related field, and have a valid NC driver's license and personal vehicle insurance coverage.
