Job Summary

The Nurse (RN) is a key member of the Assertive Community Treatment (ACT) or Family Practice team at Southeastern Integrated Care LLC, providing person-centered services to address the diverse needs of beneficiaries and help them achieve their personal goals.

Key Responsibilities
  • Medication Management: Collaborate with the psychiatrist to manage the medication system, administer, and document medication treatment.
  • Beneficiary Support: Build rapport and engagement with beneficiaries, screen and monitor for medical problems and side effects, and complete AIMS assessments.
  • Health Promotion: Engage in health promotion, prevention, and education activities to support beneficiaries' well-being.
  • Medication Adherence: Develop strategies to maximize medication adherence, including reviewing home environments and working with beneficiaries and psychiatric care providers.
  • Communication and Coordination: Communicate and coordinate services with other medical providers to ensure comprehensive care.
  • Crisis Response: Provide first-responder crisis response to beneficiaries experiencing a crisis.
  • Team Collaboration: Attend daily ACT team meetings and actively participate in discussions.
  • Documentation: Accurately document all billable encounters into Southeastern Integrated Care's EMR system within 24 hours.
Supervisory Responsibilities

The Nurse (RN) position oversees Nurse (LPN) staff, providing guidance and support to ensure the delivery of high-quality care.

Qualifications
  • Education: Associate's Degree of Nursing (RN)
  • Experience: Minimum one year of experience working with adults with serious mental illness
  • Skills: Working knowledge of psychiatric medications, ability to empathetically interact with beneficiaries and their families, strong clinical skills, and ability to operate independently or with minimal supervision.
Certifications and Licenses
  • NC Nursing License: Valid NC Nursing License or Compact License that allows practice in NC
  • Driver's License: Copy of a valid, unrestricted NC Driver's license, proof of insurance
