Job Title: Certified Peer Support Specialist

Job Summary:

We are seeking a Certified Peer Support Specialist to join our team at Southeastern Integrated Care LLC. As a Peer Support Specialist, you will play a vital role in providing coaching, mentoring, and consultation to individuals in recovery, promoting self-advocacy, self-direction, and wellness self-management.

Key Responsibilities:

  • Provide individualized support to clients, focusing on their unique needs and goals.
  • Assist clients in developing and implementing a Mental Health Wellness Recovery Action Plan.
  • Link clients to community resources and provide case management services.
  • Document direct service delivery according to Medicaid and CSEUC standards.
  • Collaborate with the interdisciplinary healthcare team to promote a culture of recovery and empowerment.

Requirements:

  • NC Certified Peer Support Specialist certification.
  • Valid driver's license.
  • Ability to read a medical record and find pertinent information quickly.
  • Ability to write complete service notes.
  • Effective communication and interpersonal skills.

Work Environment:

The work environment is primarily in the client's living environment and in the community, with occasional exposure to cigarette smoke, domestic animals/pets, and other household issues.

Physical Demands:

The physical demands of this job include standing, walking, and lifting up to 20 pounds, with occasional lifting up to 50 pounds.
