Southeastern Integrated Care LLC is seeking a highly skilled and compassionate Case Manager - Community Support Team to join our dynamic team. As a key member of our Community Support Team, you will play a vital role in providing high-quality care and support to individuals struggling with behavioral health disorders, substance use disorders, and chronic medical conditions.
Job SummaryThe primary responsibility of this position is to provide Community Support Team services, including direct and indirect interventions, to individuals with complex and extensive treatment needs. You will work collaboratively with a team of professionals to develop and implement comprehensive service plans, provide psycho-education, and assist with crisis interventions.
Key Responsibilities- Develop and implement Person-Centered Plans (PCPs) in collaboration with the Community Support Team Leader and other team members.
- Provide psycho-education and support to individuals and their families, as indicated in the PCP.
- Assist with crisis interventions and provide support to individuals in crisis situations.
- Coordinate services and linkages to community resources, including medical and non-medical providers.
- Monitor and document the status of individuals' progress and the effectiveness of strategies and interventions outlined in the PCP.
- Participate in a first-responder on-call system and coordinate resources according to individual need.
- Master's degree in a human service field or a related field, or a Bachelor's degree in a human service field with two years of experience in the adult MH/SA population.
- Valid NC driver's license and personal vehicle insurance coverage.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and community resources.
