Role Summary

The Family Support Case Manager plays a crucial role in assisting mothers with dependent children who are engaged in substance use disorder treatment. This position encompasses three primary functions:
  1. Facilitating and coordinating parenting and family programming in collaboration with the Family Program Team.
  2. Providing skill development and support for daily family activities with mothers and their children.
  3. Ensuring effective communication and coordination of care among all services utilized by the family.
Key Responsibilities
  • Embrace and uphold SAMSHA's recovery principles, fostering a supportive environment for personal growth and wellness.
  • Assist families in securing safe housing and provide guidance in housing searches.
  • Lead and co-facilitate educational support groups, focusing on parenting and treatment.
  • Organize and implement children's programming in collaboration with the MARFU team.
  • Evaluate and support mothers in enhancing their parenting and recovery skills.
  • Collaborate on individualized goal plans for mothers and families.
  • Facilitate skill-building sessions in areas such as parenting, stress management, and problem-solving.
  • Conduct most client interactions within participant residences.
  • Provide transportation assistance for appointments and events as necessary.
  • Support mothers in addressing their children's needs, including education, health, and behavioral development.
  • Engage in the division's Trauma-Informed Care Initiative and apply evidence-based practices.
  • Promote health and wellness activities within a holistic care framework.
  • Contribute positively to team dynamics, fostering a collaborative work environment.
Qualifications

Essential
  • Four years of experience in providing services to women facing chemical dependency challenges, or an Associate's degree with two years of experience, or a Bachelor's degree in Social Work, Chemical Health, Human Services, or a related field.
  • Proven experience in group facilitation.
  • Commitment to working in a diverse, fast-paced service environment.
  • Experience in delivering culturally responsive services.
  • A minimum of one year of sobriety from alcohol and chemical dependencies.
Preferred
  • Licensure as an Alcohol and Drug Counselor (LADC) is a plus.
  • Experience in a behavioral health setting.
  • Background in culturally diverse environments.
  • Familiarity with programs serving individuals with dual diagnoses.
