Senior Case Manager - Aging & Disability Services

Salary Range: $50,000 - $58,000 per year

Position Overview: Catholic Charities is seeking a committed and meticulous professional to take on the role of Senior Case Manager in our Aging and Disability Services division. The perfect candidate should possess compassion, a trauma-informed approach, and cultural sensitivity.

This role requires collaboration with clients to develop and oversee personalized service plans, conducting thorough intake evaluations, identifying available community resources, and leveraging Catholic Charities services. Successful candidates will maintain detailed case documentation, provide continuous support to clients, track progress, and modify service plans as necessary.

Core Responsibilities:

  • Conduct initial and periodic assessments while delivering case management services to a diverse clientele.
  • Design and uphold service plans customized to each client's requirements, utilizing both internal and external resources.
  • Advocate for clients to secure services and benefits, assisting with necessary documentation.
  • Collaborate with community organizations to ensure clients receive essential support and follow-up.
  • Guarantee comprehensive and precise documentation of cases in accordance with organizational standards.
  • Coordinate with supervisors and team members to ensure optimal client care.
  • Employ various strategies to foster personal development and mitigate crises.
  • Facilitate group or individual sessions to provide education, skills training, and support.
  • Offer mentorship to fellow Case Managers as required.
  • Transport clients in line with program guidelines.
  • Perform additional duties as assigned.

Qualifications:

  • A Bachelor's or Master's degree in social work, psychology, sociology, human services, or a related field is preferred.
  • Alternatively, a Bachelor's degree in any discipline with at least one year of relevant experience.
  • Must pass required background checks and adhere to social work standards.
  • Additional training or licensure may be necessary based on program requirements.
  • Ability to engage with diverse and low-income populations, including those facing challenges such as homelessness or mental health issues.
  • A valid driver's license and current insurance for client transportation, if necessary.
