We are in search of a Patient Care Coordinator to join our dynamic team at AndHealth. In this pivotal role, you will assist patients in navigating their healthcare journey, aiming to achieve outstanding health outcomes and ensuring they receive the necessary support for a healthier lifestyle.

Key Responsibilities:

  • Prepare examination rooms for patient consultations, ensuring they are adequately stocked and sanitized.
  • Manage patient communications, including receiving and directing test results for healthcare provider assessment, while gathering information on patient medications.
  • Assist in patient intake, monitor vital signs, and evaluate patient needs to relay information to the healthcare provider.
  • Support healthcare providers during patient procedures as required.
  • Communicate effectively in both written and verbal formats with patients, their families, colleagues, and external parties.
  • Complete necessary documentation for patients as needed.
  • Ensure accurate and timely documentation in the Electronic Health Record (EHR).
  • Conduct laboratory tests as instructed by the healthcare provider, manage specimen preparation, operate laboratory equipment, and maintain compliance with regulatory standards.
  • Reconcile billing from external laboratories by comparing daily logs with patient records.
  • Coordinate scheduling for external diagnostic tests and maintain effective communication with patients.
  • Facilitate patient referrals as necessary.
  • Maintain an inventory of medications.
  • Perform administrative duties as required.
  • Adhere to all policies and procedures of the healthcare organization.
  • Other duties as assigned based on operational needs.

Qualifications:

  • A Bachelor's degree is preferred.
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) credentials are preferred.
  • Certification in Basic Life Support (BLS) from the American Heart Association is required or must be obtained.

Desired Skills:

  • Experience in an outpatient or ambulatory care setting is preferred.
  • Proficiency in Microsoft Office applications.
  • Ability to navigate Electronic Medical Records (EMR) and Electronic Health Records (EHR) systems.
  • A strong sense of ownership and initiative to enhance each patient's experience.
  • Exceptional written and verbal communication skills.
  • Attention to detail and the ability to manage multiple tasks simultaneously.
  • A collaborative spirit and a servant-leadership approach in interactions.
  • Comfort with technology and a willingness to learn new systems.
  • A passion for teamwork across various organizations.

Additional Requirements:

  • Proof of a negative Tuberculosis (TB) skin test within the last year.
  • Documentation of immunity to Measles, Mumps, and Rubella (MMR) or a blood titer.
  • Proof of Varicella vaccination or a blood titer.
  • Completion of the Hepatitis B vaccination series.
  • Influenza vaccination is required.
  • COVID-19 vaccination is required.
