Position Overview:
Under the guidance of the Access: Drug User Health Program Manager and the Medical Director of Public Health, the Clinic Navigator plays a pivotal role in two main areas: facilitating the walk-in medical clinic for individuals who use drugs (PWUD), and ensuring comprehensive care coordination and follow-up for clinic patients at Access's Cambridge Location.

Key Responsibilities:

  • Facilitate Walk-In Medical Clinic:
    Prepare for patient visits by managing client registration and insurance verification in the Electronic Medical Record system. Conduct vital sign assessments, maintain cleanliness in examination rooms, and assist medical providers with patient care. Ensure smooth clinic operations in a drop-in outpatient setting.
  • Care Coordination Services:
    Provide accessible services within a harm reduction framework, assisting clients in accessing medical and behavioral health care, substance use treatment, and housing referrals. Support patients in scheduling appointments and collaborate with the Harm Reduction Case Manager to address specialty care needs.
  • Harm Reduction Services:
    Deliver overdose prevention training and distribute Naloxone. Safely handle and dispose of syringes, ensuring adherence to biohazard protocols. Participate in mobile outreach as necessary and maintain a safe environment for community members.
  • Program Development:
    Engage in interdisciplinary team meetings to plan and implement new interventions. Collaborate with clinical and non-clinical providers to ensure quality care and a seamless continuum of services.
  • Compliance and Professional Development:
    Adhere to agency policies and participate in quality improvement activities. Maintain confidentiality and safety standards while attending required meetings and professional training sessions.

Qualifications:

  • High School Diploma/GED required.
  • Phlebotomy certification or willingness to obtain training.
  • Familiarity with medical terminology and electronic medical records.
  • Commitment to harm reduction strategies and advocacy for drug user health.
  • Strong organizational and communication skills.
  • Ability to manage challenging situations with professionalism.
  • Experience working with diverse populations, particularly those at risk for HIV, Hepatitis C, and STIs.

Preferred Qualifications:

  • Current CPR Certification.
  • Experience in harm reduction.
  • Completion of Nursing Assistant, Medical Assistant, or EMT Program preferred.
  • Knowledge of emergency housing options and health-related social issues faced by injection drug users.

Physical Requirements:
Ability to meet physical demands typical of an office environment, including handling objects up to 15-20 pounds and exposure to potentially hazardous materials.
