VitalCore Health Strategies is seeking a highly skilled Mental Health Professional to join our team at the Northeastern Correctional Center in Concord, MA.

About the Role:

  • As a Mental Health Professional, you will work collaboratively with a multidisciplinary team to provide proactive and compassionate care to patients.
  • You will be responsible for crisis stabilization, intakes, and ongoing evaluations, as well as providing consultation to in-house and community services.
  • Participate in monthly behavioral health calls and work collaboratively with the medical team to determine overall treatment for each individual.
  • Perform risk assessments of inmates, coordinate care with interdisciplinary teams, and develop discharge plans.
  • Assist in planning and implementing program goals and objectives.
  • Provide timely and accurate documentation of patient care.

Requirements:

  • Master's degree in a related field and licensure as a LCSW, LPC, or equivalent.
  • Minimum of one year of experience in a social work capacity in a public or private agency.
  • Ability to work independently and as part of a team.
  • Excellent communication and documentation skills.
