Registered Nurse

We are seeking a skilled Registered Nurse to join our Home Health team on a full-time basis. As a Registered Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients.
Responsibilities- Provide professional nursing care to patients in their homes, focusing on promoting health, preventing illness, and managing disease.
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address these needs.
- Collaborate with patients, families, and healthcare teams to identify and prioritize patient goals, ensuring that care is patient-centered and focused on achieving optimal outcomes.
- Administer medications, treatments, and therapies as prescribed by physicians, monitoring patients' responses and adjusting care plans as needed.
- Document patient progress, identifying areas for improvement and implementing changes to care plans as necessary.
- Associate's or Bachelor's degree in Nursing (AAS or BSN) or RN Diploma required.
- Current licensure as a Registered Nurse in the state of Michigan.
- Experience in home health or a related field preferred.
