Position Overview:

As a Care Manager RN, you will play a vital role in the healthcare continuum at Memorial Hospital Central. This position is essential in assessing, planning, implementing, coordinating, monitoring, and evaluating the services necessary to meet individual health needs. Your expertise in communication, education, and resource management will be crucial in promoting quality and cost-effective outcomes.

Key Responsibilities:
  • Conduct comprehensive assessments to identify health care needs and establish collaborative partnerships with clients.
  • Advocate for clients by providing education on treatment options, community resources, and psycho-social concerns, facilitating goal setting and self-care development.
  • Ensure effective communication and coordination among healthcare team members, actively involving clients in the decision-making process to reduce service fragmentation.
  • Utilize critical thinking and decisive judgment within your scope of practice, demonstrating the ability to work independently.

Qualifications:
  • Graduate of an accredited nursing program.
  • Minimum of 2 years of nursing experience, with a preference for acute care and case management experience.
  • Preferred qualifications include a Bachelor's degree in Nursing and certifications such as Basic Life Support (BLS) and Certified Case Manager (CCM).
