Key Responsibilities

Essential Duties:

1: Deliver advanced nursing services that require critical analysis and independent judgment. You will manage and coordinate patient care in an outpatient setting, utilizing both in-person consultations and telehealth technologies. Responsibilities include:

  • Conducting nursing triage to assess care needs.
  • Providing health maintenance, disease prevention, and patient education.
  • Managing referrals and coordinating care across the healthcare continuum.
  • Documenting all patient interactions in electronic health records.

2: Perform clinical procedures such as catheterizations, ECGs, and wound care, ensuring accurate assessment and response to patient needs. You will:

  • Administer medications safely and effectively.
  • Monitor patient responses to treatments and adjust care plans accordingly.
  • Educate patients about procedures and post-care instructions.

3: Offer health education and coaching to patients and their families, ensuring understanding and adherence to care plans. This includes:

  • Providing tailored health instructions based on individual needs.
  • Evaluating patient comprehension and adjusting education methods as necessary.
  • Facilitating support groups and educational programs.

4: Engage in professional development through continuing education and quality improvement initiatives. Responsibilities include:

  • Participating in training and educational programs.
  • Implementing evidence-based practices in daily nursing activities.
  • Contributing to the development of clinic policies and procedures.

5: Mentor and oversee junior staff and nursing assistants, ensuring adherence to best practices and standards of care. Duties include:

  • Providing training and orientation for new staff.
  • Participating in peer review and quality assurance activities.
  • Encouraging professional growth and development among team members.

6: Execute approved clinical protocols to provide efficient patient care. This involves:

  • Assessing patients and determining appropriate care pathways.
  • Documenting encounters and coordinating with providers for follow-up care.

7: Perform additional duties as assigned to support the healthcare team and enhance patient care delivery.

Qualifications
Eligibility:

Must be a U.S. citizen with a minimum of one year of licensed experience as a registered nurse in an ambulatory care setting. A valid nursing license in a U.S. state or territory is required.
