Position Overview:
As a Clinical Nurse in the Ambulatory Services sector, you will play a vital role in delivering high-quality patient care. This position is ideal for individuals who thrive in a dynamic healthcare environment and are committed to enhancing patient outcomes through effective care coordination and education.

Key Responsibilities:

  • Patient Assessment and Care Coordination: Conduct comprehensive telephone and clinical assessments, prioritize patient care needs, and facilitate care coordination among multidisciplinary teams. Evaluate patient progress towards wellness and provide essential education to enhance patient understanding and compliance.
  • Clinical Support: Act as a relief nurse, triaging walk-in patients to assess their conditions and determining the necessity for provider intervention. Maintain accurate documentation of patient interactions and care provided, ensuring compliance with healthcare standards.
  • Medication Administration: Prepare and administer medications via various routes, ensuring adherence to established protocols. Instruct patients on self-care and medication management, reinforcing healthcare provider instructions to promote health maintenance.
  • Patient and Staff Education: Facilitate educational initiatives for patients and staff, addressing topics related to acute and chronic illnesses, wellness, and preventive health measures. Assess educational needs and evaluate the effectiveness of educational programs.
  • Referral Coordination: Manage patient referrals to support services within the community and assist patients in adhering to their medical treatment plans through follow-up communications.
Qualifications:

US Citizens

Basic Requirement:
A graduate or higher-level degree in nursing from an accredited institution is required. This education must be recognized by the appropriate accrediting bodies at the time of graduation.

Licensure:
A current, unrestricted license as a professional nurse in the United States is mandatory, along with successful completion of the National Council Licensure Examination.

Foreign Degree:
For candidates with degrees from foreign nursing schools, official certification from the Commission on Graduates of Foreign Nursing Schools is required.
