Position Overview

As a Patient Care Facilitator at EngageMED Inc., you will play a crucial role in enhancing patient care and coordination within our healthcare team.

  • Collaboration: Work closely with healthcare providers and team members to identify specific patient populations requiring targeted care.
  • Assessment: Evaluate the healthcare, educational, and psychosocial needs of patients and their families using standardized assessment tools.
  • Care Monitoring: Partner with providers and patients to track the progress of individualized care plans and implement necessary interventions.
  • Self-Management Support: Empower patients to take charge of their health and achieve independence through effective self-management strategies.
  • Clinical Interventions: Facilitate clinical actions based on established protocols and evidence-based guidelines.
  • Team Approach: Promote a collaborative environment that includes patients and their families as integral members of the care team.
  • Follow-Up Care: Ensure timely follow-up with patients transitioning from emergency or inpatient settings, including reviewing discharge instructions and coordinating further care.
  • Communication Skills: Exhibit strong critical thinking and relationship-building communication skills, both written and verbal.
  • Documentation: Maintain accurate records of all patient outreach and care coordination activities.
  • Process Improvement: Collaborate with practice leadership to evaluate and enhance care coordination processes continuously.

Qualifications and Education Requirements
  • Current Medical Assistant Certification and/or Registration or Current LPN license.
  • Experience in patient care within primary or ambulatory care settings within the last five years.
  • Strong analytical skills and critical thinking abilities.
  • Excellent verbal and written communication capabilities.
  • Proficient interpersonal and facilitation skills.
  • Ability to adapt to changing priorities and contribute effectively as a team member.
  • Solid time management, prioritization, and organizational skills.
Technical Skills
  • General computer proficiency and ability to utilize various software applications.
