Nurse Practitioner Role in Hospice and Palliative Care

About the Role

Banner Health is seeking an experienced Acute Care Nurse Practitioner to join our team in providing high-quality palliative care to patients in need. As a key member of our interdisciplinary team, you will work closely with physicians, social workers, and chaplains to manage patients and provide compassionate care in challenging situations.

Responsibilities
  • Provide clinical management of patients with serious illnesses, including malignant pain management and complex symptom control
  • Lead discussions with patients and their families about goals of care and advance care planning
  • Collaborate with physicians and other clinicians to develop and implement patient care plans
  • Participate in interdisciplinary team meetings to discuss patient progress and develop strategies for improvement
Requirements
  • Current/Eligible (ACNP) Certification and Arizona License
  • CHPN/ACHPN preferred
  • Current CPR and BLS
  • Current prescribing privileges including a DEA license for controlled substance
  • Patient care experience preferred in Palliative, Hospice, Geriatrics, or End-of-Life Care
  • Excellent communication and team skills
