The LPN plays a crucial role in assessing patient and family needs, delivering compassionate care in alignment with physician directives and the comprehensive care plan established by the interdisciplinary team. Reporting to the Clinical Coordinator and Program Director, the LPN ensures the highest standards of comfort and health for patients and their families, adhering to the policies and procedures of Dependable Hospice Care.

We are in search of a motivated, empathetic, and proactive individual to fill the role of Licensed Practical Nurse (LPN) in the Tucson area.

Qualifications:

  • Current licensure as an LPN/LVN in Arizona
  • Availability for some after-hours and weekend shifts
  • Strong verbal and written communication skills, along with organizational abilities
  • Valid driver's license with an insured vehicle that meets state and organizational standards
  • Current BLS certification required
  • Proven nursing skills, ability to manage family emotional dynamics, and respect for diverse lifestyles
  • Possession or ability to obtain an Arizona class 1 fingerprint clearance card
  • Experience in hospice care is advantageous

Key Responsibilities:

  • Assessing patient and family needs while providing supportive care in accordance with physician orders and the interdisciplinary care plan
  • Collaborating with the assigned Nurse Case Manager to take responsibility for the nursing process
  • Initiating communication with attending physicians, hospice staff, and other agencies to ensure optimal patient care and resource utilization
  • Maintaining consistent communication with the Nurse Case Manager, Program Director, and Medical Director
  • Gathering data on physical, psychological, social, and spiritual factors affecting patient and family health, and reporting findings to the assigned Registered Nurse Case Manager
