Palliative Care Nurse Practitioner

Position Summary:
- The Nurse Practitioner will conduct consultation visits for palliative care, facilitate goals of care discussions, manage symptoms, and perform Hospice face-to-face evaluations as requested by licensed practitioners.
- Additional responsibilities include conducting comprehensive physical assessments, documenting medical histories, ordering lab tests, performing diagnostic and therapeutic procedures, prescribing necessary treatments and medications, offering health promotion advice, managing acute and chronic health conditions, and coordinating referrals to other healthcare professionals as needed.
- The supervising physician will oversee the Nurse Practitioner's activities, ensuring collaboration, consultation, and evaluation of medical actions as outlined in the collaborative agreement, maintaining accountability for the care provided by the Nurse Practitioner. In the absence of the supervising physician, an alternate will assume these responsibilities. The Nurse Practitioner will ensure that all collaborative agreements and state documentation are up to date with all supervising physicians.
Coverage Protocols:
Scope of Practice and Limitations:
- The Nurse Practitioner will operate within the defined scope of practice as specified in their job description, adhering to all applicable home health, palliative, and hospice regulations.
- Consultation with the supervising physician or an alternate will be necessary under specific circumstances.
- Prescriptive Authority: The Nurse Practitioner will comply with all relevant regulations, including maintaining necessary licenses for prescribing and an active DEA registration. They are authorized to prescribe medications in accordance with their established scope of practice and applicable regulations.
- Review of Prescribing Practices: The Nurse Practitioner must submit documentation in line with hospital guidelines for review by the licensed practitioner within a specified timeframe. This review will include a random sampling of records to assess prescribed medications.
- Patient Care Activities: It is the Nurse Practitioner's responsibility to ensure that all procedures and competencies are performed in compliance with relevant regulations, including physician supervision requirements.
- Ordering Medical Services: The Nurse Practitioner may order medical devices, biologicals, respiratory therapy, physical therapy, home health services, orthotics, prosthetics, and hospice care as appropriate, in accordance with their defined scope of practice.
- No additional limitations are imposed on the Nurse Practitioner.
- Documentation Responsibilities: The Nurse Practitioner is accountable for thorough documentation of all patient interactions, in compliance with applicable regulations. All encounters should be documented and closed by the end of the business day in which they occurred, not exceeding three calendar days.
- Deviations from established protocols and guidelines.
- Clinical practice challenges encountered.
- Acute deterioration of a patient's condition or emergence of a medical emergency.
