Compassionate Nurse Practitioner for Hospice Care

Overview:

The Nurse Practitioner will enhance the services provided by hospice physicians and other healthcare professionals. In the absence of the Hospice Medical Director, this role may serve as the medical representative for the Interdisciplinary Group (IDG), delivering essential medical guidance, including care in home settings, inpatient care, emergency interventions, and respite services related to terminal illnesses. The Nurse Practitioner will be accountable for the medical care of hospice patients and will act as the primary medical resource for the IDG, ensuring medical coverage and support beyond standard business hours, as well as conducting patient visits for assessments and necessary interventions.

Key Responsibilities:

  1. Oversee the organization and execution of Face-to-Face evaluations.
  2. Contribute expertise during regular interdisciplinary team meetings, including the assessment and discussion of Face-to-Face findings as needed.
  3. Ensure adherence to established policies and procedures.
  4. Maintain communication with the Medical Director regarding Face-to-Face evaluations and the necessity for further assessments or second opinions.
  5. Provide constructive feedback and support to clinical staff in collaboration with other management team members.
  6. When applicable, assist in the preparation of performance evaluations for staff in partnership with relevant management personnel.
  7. Identify educational needs for staff and aid in the creation of training programs for the interdisciplinary team.
  8. Support education and training initiatives.
  9. Assist with pre-employment assessments, testing, and physical examinations as required.
  10. Complete specific tasks as assigned by the Director.
  11. Evaluate the nursing requirements of hospice patients during Face-to-Face meetings.
  12. Address inquiries from hospice patients and their families regarding care provision.
  13. Uphold confidentiality and protect all patient-related information in accordance with HIPAA regulations.
  14. Communicate with the Director and Medical Director if unable to fulfill a scheduled Face-to-Face meeting.
  15. Maintain professional skills and knowledge.
  16. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.

Job Requirements:

Physical Demands: This position requires moderate physical effort to engage with and assess active patients receiving hospice care. Candidates must possess adequate sight and hearing abilities or utilize appropriate adaptive devices to perform essential job functions. Evidence of annual TB testing and compliance with state-required health assessments is necessary.

Mental Demands: The role necessitates the ability to work independently, make informed decisions based on assessments and available data, and respond appropriately. Flexibility, creativity, and strong interpersonal skills are essential. Candidates must manage mental and emotional stress effectively and demonstrate emotional resilience.

Work Environment: Candidates should be prepared to tolerate exposure to various elements, including but not limited to odors, bodily fluids, and adverse environmental conditions.

Transportation Requirements: A valid driver's license, auto liability insurance, and reliable transportation are mandatory.
