Overview:

The primary objective of this role is to enhance the services provided by hospice physicians and Nurse Practitioners. In the absence of the Hospice Medical Director, the Nurse Practitioner may serve as the medical representative of the Interdisciplinary Group (IDG), delivering essential medical oversight. This includes managing care in various settings such as home care, inpatient care, emergency situations, and respite care for patients with terminal conditions.

Key Responsibilities:

  1. Oversee the organization and execution of Face-to-Face evaluations.
  2. Contribute expertise during interdisciplinary team meetings, including the assessment and discussion of Face-to-Face findings.
  3. Ensure adherence to established policies and procedures.
  4. Maintain communication with the Medical Director regarding Face-to-Face evaluations and the necessity for additional assessments or second opinions.
  5. Provide constructive feedback and support to clinical staff in collaboration with management.
  6. Assist in performance evaluations for staff, working with management as needed.
  7. Identify educational needs for staff and aid in developing training programs for the interdisciplinary team.
  8. Support education and training initiatives.
  9. Assist with pre-employment assessments and physical examinations as required.
  10. Complete specific tasks as assigned by the Director.
  11. Evaluate the nursing requirements of hospice patients during Face-to-Face meetings.
  12. Address inquiries from patients and their families regarding care provisions.
  13. Uphold confidentiality and protect all patient-related information in accordance with HIPAA regulations.
  14. Notify the Director and Medical Director if unable to attend a scheduled Face-to-Face meeting.
  15. Continuously enhance skills and knowledge relevant to the role.
  16. Engage in Quality Assurance and Performance Improvement (QAPI) initiatives.

Job Requirements:

Physical Demands: This position requires moderate physical activity to interact with and access patients receiving hospice care. Candidates must possess adequate sight and hearing capabilities or utilize appropriate adaptive devices to fulfill the essential duties of the role. Documentation of annual TB testing and other state-mandated examinations is required.

Cognitive Requirements: The ability to work autonomously, make informed decisions based on assessments and available data, and respond appropriately is essential. Candidates should exhibit flexibility, creativity, and strong interpersonal skills. The role demands the capacity to manage mental and emotional stress while demonstrating emotional resilience.

Work Environment: Candidates must be able to withstand exposure to various elements, including but not limited to odors, bodily fluids, and adverse environmental conditions.

Transportation Requirements: A valid driver's license, auto liability insurance, and reliable transportation are mandatory.
