Job Description

Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Dependable Hospice and Palliative Care. As an LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in the Tucson, AZ area.

Key Responsibilities:

  • Assess Patient Needs: Identify the physical, psychological, social, and spiritual needs of patients and families, and provide supportive care in accordance with the attending physician's orders and the interdisciplinary group plan of care.
  • Collaborate with Healthcare Team: Work closely with the assigned Nurse Case Manager, Program Director, and Medical Director to coordinate optimal care and use of resources for patients and families.
  • Communicate Effectively: Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director to ensure seamless care coordination and patient satisfaction.
  • Provide Emotional Support: Offer emotional support and comfort to patients and families, and provide education on disease management and end-of-life care.

Requirements:

  • Current LPN License: Must be currently licensed as an LPN/LVN in the State of Arizona.
  • Experience: Some after-hours and weekend coverage is required.
  • Communication Skills: Demonstrates good verbal and written communication, and organized skills.
  • Transportation: Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Certifications: Possesses and maintains current BLS certification.
  • Clearance: Currently possess or able to obtain an Arizona class 1 fingerprint clearance card.

Preferred Qualifications:

  • Hospice Experience: Previous experience in hospice care is a plus.
