ROLE SUMMARY
The Advanced Practice Provider in Palliative Care is tasked with alleviating the symptoms associated with patients' chronic progressive illnesses while preventing unnecessary hospital admissions through effective triage and care coordination. This mobile role emphasizes the importance of delivering comprehensive care directly to patients in their homes.
Key Responsibilities:
- Deliver advanced primary and palliative care services to patients in their residences.
- Formulate treatment strategies aimed at effective symptom management.
- Provide on-call assistance and collaborate closely with the triage team to avert hospitalizations and enhance patient support.
- Engage with healthcare partners and referral sources to nurture and maintain community relationships.
- Coordinate with the patient's care team and referral sources to optimize patient care and minimize readmissions.
- Facilitate comfort measures, including pain management, home health services, and spiritual care.
- Support end-of-life discussions and assist with hospice transitions when necessary.
*Flexible hours available, Monday through Friday.*
QUALIFICATIONS
- Master's degree in nursing from an accredited institution with a current professional nursing license or graduation from an ARC-PA accredited physician assistant program.
- Board certification as an advanced nurse practitioner or Physician Assistant.
- Active state license in good standing.
- Current DEA registration.
- CPR certification.
- A minimum of 2 years of healthcare experience in settings such as hospitals, outpatient clinics, home health, hospice, or specialty care.
- Must possess a valid driver's license and have access to a reliable vehicle, ensuring compliance with state and organizational insurance requirements.
