POSITION DESCRIPTION:
The role of the Community Health Worker involves establishing vital connections between community members, the healthcare system, and social service resources to enhance health outcomes and life circumstances. This position aids individuals in navigating the complexities of physical health, behavioral health, and social services by addressing social determinants of health and acting as a liaison among individuals, healthcare providers, and social service organizations. The Community Health Worker operates in both clinical and community environments, including client residences.
KEY RESPONSIBILITIES:
- Conduct thorough assessments for assigned clients, provide appropriate referrals, and follow up on outcomes and subsequent steps.
- Engage with clients through various communication methods (phone, text, email, home visits) to discuss health service needs and schedule necessary appointments.
- Assist clients in understanding and accessing health information from their medical and social service providers.
- Support clients in completing necessary paperwork related to health and social determinants of health, including applications and registration forms.
- Perform home visits as required to provide education, follow-up, and support.
- Collaborate with both internal and external partners to ensure integrated patient care, care coordination, and patient engagement.
- Participate in community events to promote health services and encourage healthy behaviors.
- Maintain accurate and current client records by documenting services and interactions in the designated database system.
- Complete community health worker training and any additional assigned training.
- Carry out other responsibilities as assigned.

EDUCATION AND EXPERIENCE:
A high school diploma or GED is required, along with a valid driver's license, insured and reliable transportation, and significant community engagement experience. Experience in community outreach, peer counseling, health promotion, customer service, healthcare/medical office roles, and/or data entry is preferred.

PHYSICAL DEMANDS:
This position requires light physical activity related to non-strenuous daily administrative tasks. The ability to lift up to thirty (30) pounds is necessary.
WORK SCHEDULE:
The standard work hours for this position are Monday through Friday during the day. Attendance at off-site meetings may be required, and local travel is necessary. A valid driver's license and insured vehicle are required. Occasional out-of-town travel may also be necessary.
