Optum is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Salem/Eugene, OR. As a clinician-led care organization, we're creating a seamless health journey for patients across the care continuum.

About the Role

In this role, you'll have the opportunity to provide high-quality care to patients in assisted living and other community-based settings. Our care model focuses on palliative care with internal medicine, and you'll work as part of an interdisciplinary team to address the physical, mental, and social needs of our patients.

Responsibilities
  • Conduct comprehensive assessments and develop personalized care plans
  • Manage patients through acute medical situations and chronic condition exacerbations
  • Collaborate with the care team to ensure seamless transitions and coordination
  • Provide education and support to patients and caregivers
  • Document medical information accurately and efficiently

Requirements
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (NPs) or accredited Physician Assistant degree program (PAs)
  • Active and unrestricted licensure in the state of practice
  • Board certification in a relevant specialty (NPs) or National Commission on Certification of Physician Assistants (NCCPA) certification (PAs)
  • Ability to obtain DEA licensure/prescriptive authority, if required
Preferred Qualifications
  • 1+ years of clinical experience in senior living, geriatric medicine, or home care
  • Experience in meeting the healthcare needs of patients with complex behavioral, social, and/or functional needs
