Senior Community Care Clinician Opportunity

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Portland, OR. As a clinician-led care organization, we are creating a seamless health journey for patients across the care continuum.

About the Role

In this role, you will provide comprehensive assessments, manage patients through acute medical situations, and ensure accurate and complete documentation. You will also communicate and collaborate with the interdisciplinary care team, conduct advanced illness and advanced care planning conversations, and provide patients and caregivers with counseling and education.

Key Responsibilities
  • Conduct comprehensive assessments and develop patient care plans
  • Manage patients through acute medical situations, including testing, diagnostics, and interventions
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education
Requirements

To be successful in this role, you will need:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants)
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date
  • Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants)
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and a variety of dwelling conditions
