Senior Community Care Opportunity

Optum Home and Community Care is seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Portland, OR. As a clinician-led care organization, we are creating a seamless health journey for patients across the care continuum.

Key Responsibilities:
  • Conduct comprehensive assessments and manage patients through acute medical situations or exacerbations of chronic conditions.
  • Ensure accurate and complete documentation, including ICD-10 conditions, and communicate with the interdisciplinary care team.
  • Provide patients and caregivers with counseling and education, and facilitate agreement and implementation of the patient's plan of care.
  • Coordinate care as patients transition through different levels of care and care settings.

Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants).
  • Active and unrestricted license in the state of practice, or ability to obtain by start date.
  • Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants).
Preferred Qualifications:
  • New graduates are encouraged to apply.
  • 1+ years of clinical experience in senior living, geriatric medicine, or home care.
  • Experience in meeting the healthcare needs of patients with complex behavioral, social, and/or functional needs.
