Nurse Practitioner or Physician Assistant

Optum Home and Community Care is seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Portland, OR. As a clinician-led care organization, we are creating a seamless health journey for patients across the care continuum.
Key Responsibilities:- Conduct comprehensive assessments and manage patients through acute medical situations or exacerbations of chronic conditions.
- Ensure accurate and complete documentation, including ICD-10 conditions, and communicate with the interdisciplinary care team.
- Provide patients and caregivers with counseling and education, and facilitate agreement and implementation of the patient's plan of care.
- Coordinate care as patients transition through different levels of care and care settings.

- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants).
- Active and unrestricted license in the state of practice, or ability to obtain by start date.
- Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants).
- New graduates are encouraged to apply.
- 1+ years of clinical experience in senior living, geriatric medicine, or home care.
- Experience in meeting the healthcare needs of patients with complex behavioral, social, and/or functional needs.
