About the RoleThis is a challenging and rewarding role that requires a high level of clinical expertise, strong communication skills, and a passion for delivering exceptional patient care. As a Clinical Nurse Practitioner or Physician Assistant, you will be responsible for providing high-quality, patient-centered care to our senior patients, working collaboratively with our interdisciplinary team to develop and implement individualized care plans, and participating in quality improvement initiatives to enhance patient outcomes.
Key Responsibilities- Provide comprehensive, patient-centered care to senior patients, including health assessments, diagnosis, and treatment planning.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Participate in quality improvement initiatives to enhance patient outcomes and improve the overall quality of care.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Stay up-to-date with the latest clinical guidelines, research, and best practices in senior care.

To be considered for this role, you must have:
- A current license to practice as a Clinical Nurse Practitioner or Physician Assistant in the state of Texas.
- At least one year of clinical experience as a Clinical Nurse Practitioner or Physician Assistant in a senior care setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
