As a Nurse Practitioner or Physician Assistant on our Senior Community Care team, you'll play a vital role in shaping the future of healthcare.

Key Responsibilities:
  • Conduct comprehensive assessments to identify patients' needs and develop personalized care plans
  • Manage patients through acute medical situations, chronic conditions, and short-term rehabilitation episodes
  • Ensure accurate and complete documentation, including ICD-10 conditions, to support high-quality care
  • Communicate and collaborate with interdisciplinary care teams to deliver seamless care
  • Provide patients and caregivers with counseling and education to promote healthy behaviors and outcomes

Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants)
  • Active and unrestricted license in the state of practice or ability to obtain by start date
  • Board certification through the American Academy of Nurse Practitioners or American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants)
