Nurse Practitioner or Physician Assistant

As a Nurse Practitioner or Physician Assistant on our Senior Community Care team, you'll play a vital role in shaping the future of healthcare.
Key Responsibilities:- Conduct comprehensive assessments to identify patients' needs and develop personalized care plans
- Manage patients through acute medical situations, chronic conditions, and short-term rehabilitation episodes
- Ensure accurate and complete documentation, including ICD-10 conditions, to support high-quality care
- Communicate and collaborate with interdisciplinary care teams to deliver seamless care
- Provide patients and caregivers with counseling and education to promote healthy behaviors and outcomes

- Board certification through the American Academy of Nurse Practitioners or American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants)
