Key Responsibilities:
  • Conduct comprehensive assessments to identify patients' needs and develop personalized care plans.
  • Manage patients through acute medical situations, chronic conditions, and rehabilitation episodes.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and counseling to patients and caregivers, empowering them to take control of their health.
  • Document patient information accurately and efficiently, adhering to regulatory requirements and CMS guidelines.

Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (NPs) or accredited Physician Assistant degree program (PAs).
  • Active and unrestricted licensure in the state of practice or ability to obtain by start date.
  • Board certification through the American Academy of Nurse Practitioners or American Nurses Credentialing Center (NPs) or National Commission on Certification of Physician Assistants (PAs).
  • Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling for physical assessments.
Preferred Qualifications:
  • New graduates encouraged to apply.
  • 1+ years of clinical experience in senior living, geriatric medicine, or home care.
  • Experience in meeting the healthcare needs of patients with complex behavioral, social, and/or functional needs.
